38% of Physicians Don't Contact Chronic Medicare Patients Between Visits


SmartCCM recently conducted a survey on caring for patients with chronic conditions. Here are some key findings from the report:

·         63% of PCPs cite the lack of time to provide extra guidance for chronic care patients as a major challenge.

·         Patients with 2 or more chronic conditions make up more than two-thirds of the Medicare population.

·         46% of PCPs don't know they can be reimbursed for non-face time with patients with 2+ chronic conditions.

·         Physicians say 75% of Medicare patients with 2+ chronic conditions are "somewhat compliant" with care.

·         38% of physicians do not contact their Medicare patients with 2+ chronic conditions between office visits.

·         The complexity of managing multiple conditions is a top challenge for 56% of physicians.
