healthcare data breaches in 2015 with a combined loss of over 112 million
records.

The top 10 data
breaches accounted for 111 million lost, stolen or inappropriately disclosed
records.
·
4 of the top 6
breached were Blue Cross Blue Shield organizations.
·
38% of the incidents
were reported as "Unauthorized Access/Disclosure."
·
9 of the top 10
breaches were reported as a "Hacking/IT Incident."
·
The largest single
breach represented over 70% of the total records compromised.
