According to a recent study of 936 medication errrors:


  • 53.4% were drug administration errors
  • 47.2% of harmful errors were prescribing errors

470 (50.2%) of the errors were considered preventable at their origin by an existing information technology system:

  • 155 were due to IT system bypasses
  • 103 were due to insensitivity of IT alerting systems
  • 47 were due to IT alert overrides
